The short answer
An OIG exclusion check looks a provider up on the List of Excluded Individuals and Entities that the HHS Office of Inspector General publishes. This free tool takes a name, a business name or an NPI and shows whether it is excluded, needs review or has no match, with the exclusion type and the date of the list.
- No signup needed
- Read from the OIG's public list
- Shows the date of the list checked
An OIG exclusion check tells you whether a provider is on the federal exclusion list.
Type a person's name, a business name or a provider's NPI. The tool checks it against the List of Excluded Individuals and Entities that the HHS Office of Inspector General publishes, and shows whether it is excluded, needs review, was reinstated or has no match, with the exclusion type, its date and the date of the list that was read.
A name that matches is shown as needs review and never as a verdict, because a name alone does not prove it is the same person or business. An NPI that is on the list is shown as excluded. Most entries on the list carry no NPI, so an NPI that is not listed does not rule a provider out: also check by name. To keep the tool free for everyone, a check made in the last 12 hours is shown again.
The OIG replaces its list every month, by the 10th, and every answer says which date's list was read. This checks the OIG list only; state Medicaid exclusion lists are separate.

An exclusion check returns these facts, and each one answers a different question.
This is an invented result for a made-up provider, laid out the way a real one comes back. The headline is free on this page, and a free trial opens the rest.

- NPI, specialty and location
From the listing
The NPI, specialty, city and state the OIG recorded, so you can compare them with your own record.
- Birth year
On the listing
The year of birth the OIG recorded, which helps tell two people with the same name apart.

Every entry
Each other entry that matches or looks like the name, so you can rule them out one by one.
Checking a provider takes three steps and no signup.
Enter the provider
Type a first and last name, a business name or a 10-digit NPI. A lone surname is refused, because it would match hundreds of people, and an NPI with a wrong last digit is refused as mistyped.
We read the OIG list
The entry is checked against the OIG's List of Excluded Individuals and Entities, which holds more than 84,000 entries.
Read the answer
You see whether the provider is excluded, needs review or has no match, with the exclusion type, its date and the date of the list.
One check reads the OIG exclusion list and shows the outcome with the date of the list.
This is the path a single check takes. Nobody is contacted and nothing is filed.

- You see the outcomeExcluded, needs review, reinstated or no match, with the date of the list.
An exclusion type on the OIG list is a section of the Social Security Act, and each one means something different.
These are some of the codes that appear in the exclusion type column of the OIG's list, worded as the OIG's own exclusion authorities page words them. A result on this page shows the meaning in words.
| Code on the list | What it means | Kind |
|---|---|---|
| 1128a1 | Conviction of program-related crimes | Mandatory |
| 1128a2 | Conviction relating to patient abuse or neglect | Mandatory |
| 1128a3 | Felony conviction relating to health care fraud | Mandatory |
| 1128a4 | Felony conviction relating to controlled substance | Mandatory |
| 1128b1 | Misdemeanor conviction relating to health care fraud, or conviction relating to fraud in non-health care programs | Permissive |
| 1128b4 | License revocation, suspension, or surrender | Permissive |

Bulk exclusion checks run the same check on a whole list with one CSV file.
The free tool checks one provider at a time. With a 1Lookup account you upload a CSV file in the dashboard, pick the column that holds the names or NPIs and download the same file back with the result columns filled in on every row. These rows are invented.

- 3 credits for each completed check, a clear one included, and nothing for a row that is not a valid name or NPI.
- Up to 2 million rows in one CSV file.
- Up to 200 API requests per 10 seconds for each API key, and each account can make up to 1,000 requests a minute in total across all its keys.
The OIG Exclusion Check API returns the same answer as JSON from one request.
Send the provider's name, or an NPI, to the endpoint with your API key. Every completed check costs 3 credits, a clear one included, and an entry that is refused as malformed or a list that cannot be read costs 0. The outcome is in classification.outcome and the date of the list is in classification.list_updated.

}To check every provider on a list from your own software, see our OIG Exclusion Check API.
One check shows whether a provider is excluded, which kind of exclusion and since when.
A name match is flagged for review and never passed as a verdict.
A name that matches an entry comes back as needs review, because a name alone does not prove who it is. An NPI that is on the list comes back as excluded.
The exclusion type comes back in plain words.
You see License revoked, suspended or surrendered, not a section code such as 1128b4 that you would have to look up in the Social Security Act.
The date of the list is in every answer.
The OIG replaces its list every month, so each answer says which date's list was read and you can see how current the check is.
An NPI is checked before the list is read.
The last digit of an NPI is a check digit. A number that fails it was mistyped, so it is refused at once instead of being searched for.
Employers, staffing agencies, practices and credentialing teams all check providers with this tool.
Healthcare employers checking a candidate
Look a candidate up by name before an offer and see whether an entry on the OIG list matches.
Staffing agencies checking a clinician
Check a nurse, therapist or physician by name or NPI before a placement.
Practices and billing teams checking a vendor
Check a vendor, a billing company or a contractor by its business name before it is paid.
Credentialing teams checking a provider file
Look an NPI up and see whether it is on the list, then check the name too.
These are the questions people ask about the free OIG exclusion check.
Is the OIG exclusion check really free?
Yes. You can check providers without an account and without paying anything. A free 1Lookup trial removes the daily limit and adds the NPI, specialty, location and birth year of each listing, the look-alike entries and every other match.
Where does the information come from?
From the List of Excluded Individuals and Entities that the HHS Office of Inspector General publishes. The OIG replaces the list every month, by the 10th, and every result shows the date of the list that was read.
What does needs review mean?
That the name you entered matches an entry on the list. A name alone does not prove it is the same person or business, so confirm with the NPI, the location or the birth year before you act.
What does no match mean?
That nothing on the OIG list matched what you entered. It says nothing about state Medicaid exclusion lists, which are separate and not part of this tool.
Why does an NPI that is not listed not rule a provider out?
Most entries on the list carry no NPI: about 1 in 10 do. An NPI that is not listed therefore says little, and the answer tells you to also check by name.
Why was my entry refused before the check?
A single word, a name with characters other than letters, numbers, spaces and . , - ' &, and an NPI whose last digit does not match the other nine cannot be checked. The entry is refused with a clear message and does not use up a check.
Does the list show providers that were reinstated?
Normally not. The OIG removes a provider from its list when it reinstates them, so a provider who is back in good standing reads as no match. An entry that still carries a reinstatement date is shown as reinstated.
Can I check a list of providers at once?
Yes, in a free trial. Upload a CSV with a provider column or call the OIG Exclusion Check API. Each completed check costs 3 credits, a clear one included, and a row that is not a valid name or NPI costs nothing.
